I do not have a code example showing you this, but the concept would be:
1) Load the picture into a HTML5 canvas element
2) Use some code to allow drawing on top of that element
3) Use Canvas api's to export the image (maybe DataURI format)

Depending on what you need, you may be able to just develop this yourself

i tried to annotate image in online image viewer once, working well for me. i can add at least twenty types of annotations on almost any image i want.
and i can make use of preview support to view image. take one image viewer and see if it suits your needs.
